Thames Valley Family Health Team (TVFHT), one of Ontario's largest interdisciplinary primary care organizations, serving more than 160,000 patients, sought to strengthen its organizational performance and accountability framework following the development of a new strategic plan. The objective was to translate strategic priorities into measurable outcomes, establish meaningful performance indicators and equip leadership with better tools to monitor progress and drive continuous improvement.

Building on our strategic planning engagement with TVFHT, the approach integrates healthcare leadership expertise, evidence-based performance measurement and advanced AI capabilities to support the development of a comprehensive Balanced Scorecard aligned with the organization's strategic directions.

The platform brings together relevant performance data, benchmarks and AI-driven analytics to identify trends, explain performance variation and generate actionable recommendations.
